Second CCP Session Held at Security Health Plan

Security Health Plan hosted a second CCP session for disease management and case management staff on May 18th and 19th. The session followed a January 2007 CCP session funded by the State of Wisconsin in collaboration with the University of Wisconsin Medical School and Wisconsin counties. The earlier state project combined member risk assessment, disease management, and health coaching to support independence and reduce avoidable adverse health care events for at-risk community dwelling elders. Security Health Plan, rated as one of US News and World Report’s top health plans is wholly owned by Marshfield Clinic—one of the largest private, multispecialty group practices in the United States.
