Joan is the former Director of Care Transitions for Lee Memorial Health System, a 1600 bed hospital system in Fort Myers, Florida. Joan has 35 years of management experience in multiple health care arenas and an RN for 40 years. In the past year she has developed and implemented Care Transitions at 4 hospitals and has piloted a Care Transition program at the Health Park Care and Rehab Center. She has provided services to 465 patients with a 10% re admission rate at 30 days. Joan was the Director of Health Advocacy, a department developed by Joan at Lee Memorial Health System. Health Advocacy was dedicated to improving the quality of care for 8600 employees through an Integrated Absence Management Program and is the first in the country. This program housed the medical management of the Health Plan, Family Medical Leave, Short and Long Term Disability and wellness programs.
